Healthy Community Alliance

Healthy Community Alliance, Inc., located in Gowanda, NY, is a Rural Health Network and is the CBO Consortium of Upstate NY Lead Agency. The mission of Healthy Community Alliance is to improve the quality of life in rural communities through broad-based, inclusive partnerships that support wellness and prevention.

Total Senior Care

Total Senior Care is a comprehensive managed long-term care plan for adults, 55 years old or older, who wish to live at home but need assistance in order to maintain health, complete day-to-day activities and remain safe at home for as long as possible. We encourage members to take an active part in their own health care and we offer a wide range of services. The program is designed to provide each member with the very best possible care. Many of our services are provided in our program at the Total Senior Care Center, located in Olean, NY.

HomeCare & Hospice

Since 1971, HomeCare & Hospice has offered a variety of services to patients and clients in their homes or places of residence in Allegany, Cattaraugus, Genesee, and Wyoming counties in Western New York state, as well as portions of neighboring counties.

Cattaraugus County Department of Aging

The Cattaraugus County Department of the Aging provides or assures the provision of needed services to county residents who are age 60 or older. The Department was created in July of 1975, through an Act of the Cattaraugus County Legislature. Since its inception, the Department has coordinated with existing community services, planned, developed and, in some cases, administered needed programs, while also promoting new and better services for all individuals age 60 or older.

Directions in Independent Living, Inc.

Directions in Independent Living, located in Cattaraugus and Allegany counties in the Southern Tier of New York State, is driven to help individuals with disabilities live with greater dignity and independence in their personal lives and as active members in the community. Directions was begun in 1987 to provide professional support for individuals with disabilities and advocate on their behalf for equal civil rights, increased accessibility to community services and programs, and greater real independence. This on-going activism has resulted in significant changes in Olean, NY.

Interfaith Caregivers

Interfaith Caregivers is a caring community of volunteers from any faith and age whose responsibility it is to work with, serve and assist people in order that they may maintain their independence and dignity to the highest possible degree. We provide services for the Olean Association for the Blind and Visually Impaired, and primarily serve local older adults, health-impaired individuals and those who might benefit from acts of giving.

Wyoming County Office for the Aging

The Wyoming County Office for the Aging is the local area agency on aging that offers a variety of services to county residents who are age 60 and over. The agency advocates on behalf of all older persons in the service area, identifies the needs of the elderly and creates plans for meeting those needs, and administers a wide variety of Federal, State, local and private funds which support the services provided.

Ralph C. Wilson, Jr. Foundation

The Ralph C. Wilson, Jr. Foundation began operations in 2015 to continue his legacy — one of generosity and innovation, healthy risk-taking and collaboration, and an unshakable community focus. Today, his hand-picked Life Trustees have determined to focus the efforts of the foundation on Mr. Wilson’s home and adopted home regions of Southeast Michigan and Western New York. With a structure that dictates spend-down of our funds by 2035, we strive to match the urgency that people in need feel every day with community investments that consider both immediate and long-term benefit.

Health Foundation for Western and Central New York

The Health Foundation for Western and Central New York is an independent private foundation that serves 16 counties throughout Western and Central New York. To achieve outcomes that last, we work together with our community partners to develop, implement and evaluate sustainable programs that make a positive difference in the lives of thousands of young children and older adults in our regions, and support the community-based organizations that serve them.

The Philanthropic Initiative

The Philanthropic Initiative (TPI) was founded on the belief that philanthropy has the power to transform. We provide consulting and program management services to help individualsfamiliesfoundations, and companies increase the impact of their philanthropy, while also working to promote effective philanthropy through thought leadership, cutting edge research, philanthropic education programs, and field-building initiatives with key allies.

The New York Academy of Medicine

The New York Academy of Medicine (NYAM) tackles the barriers that prevent every individual from living a healthy life. NYAM generates the knowledge needed to change the systems that prevent people from accessing what they need to be healthy such as safe and affordable housing, healthy food, healthcare and more. Through its high-profile programming for the general public, focused symposia for health professionals, and its base of dedicated Fellows and Members, NYAM engages the minds and hearts of those who also value advancing health equity to maximize health for all.

International Deliverables, Inc.

International Deliverables, LLC is recognized as the industry leader in integrating creativity and Creative Problem Solving (CPS) into project management. As a master facilitator of Creative Problem Solving (CPS) and Project Management Professional (PMP), they combine research based elements of creativity with principles of project management to leverage project outcomes. International Deliverables, LLC is a certified New York State and New York City Women Business Enterprise (WBE) and a certified Federal SBA and SAM registered Woman Owned Small Business (WOSB).
